Hi Luke
I'll stand corrected on this but my understanding is Ford supplied 12 Slotters were 14 x 6 originally, any other sizes were moded originals or aftermarket. As far as the rest of your question, the centre caps are metal and come from the GS series. There are a number of versions, some fully pressed some with a plastic centre cap. I have both types.
Dress rings would have been a dealer or owner fit based on the trend from other accessories.
Cheers
G

Hey Luke, I agree with Graeme, the standard wheels look like cheese cutters, I have pressed metal chrome centre caps, also I put dress rings on mine and wider (fat) 12 slots and many people do say they look so much better.

A graeme said, but no, dress rims were not standard, and also, not a factory option. The chrome centres were pressed metal with a machined centre cap, make sure you have the rubber ring that goes around the base of the cap as well.

First bet would be to try clark rubber or any rubber seal place, failing that, GT Ford Performance and the like. It's just a U channel rubber ring, you should be able to make it yourself.
